Mission: To provide personal support services to consumers in Independent Living Situation.

Cost: $850.00 and up per month + cost of living

We work as a team to develop individual plans to meet each person’s abilities and family needs. Fees are based on that service plan.

We assist in obtaining quality living space and selection of roommates when needed in Fulton, Gwinnett, DeKalb and Cobb County.

If in CLP we help coordinate transportation or provide transportation to work, personal appointments, social activities, grocery shopping, haircuts, banking, etc.

• Medication monitoring when necessary

• Roommate and Relationship Skills

• Job Development, Training and Support

• On property staffing and a 24 hour call service

The clients in the CLP Program live at home with their parents/family members or live in an apartment, condo or house that is not inhabited solely by people with disabilities. Staff is present on most properties.
